Applications
APPLICATION PROCESS
Applications are accepted at any time and must be received and approved
before reimbursement requests will be considered. An official letter will be
sent to the approved applicant indicating that they have been approved and the
month for which they may begin applying for reimbursements. Contents of the
application package include: application form, employee compliance letter and a copy of
your provider’s State of Ohio license. In addition, the participant must provide
a copy of the provider’s State of Ohio license annually (each October). If the
participant changes their provider at any time a new application package must be
completed and submitted as described above.
Completed application packages
should be returned to: Scott McIntyre, Childcare Program Chair, USDA
Soybean Building, 1680 Madison Avenue, Wooster, OH. The Childcare Program Chair
will contact participants via letter once approval for the application has been
given.
