Providers
REGISTERED PROVIDERS Childcare Providers must be licensed by the State of Ohio. You may contact the Wayne County Human Services at 330-287-5800 for a list of licensed providers in the area. A list has been given below of providers that the OARDC Childcare Committee has at one time received a copy of their State of Ohio license. This is not an endorsement for any one of the provider's listed below. Individuals need to make their own evaluation for an appropriate childcare provider. If your licensed provider is not on the list below or was not named by the Wayne County Human Services department, simply submit a copy of their daycare license with your application.
Private Home Child Care Providers: In the event you would need to use a backup provider could you please inform your client whether the backup is a licensed provider. The OARDC Child Care Program only accepts licensed providers. You also do not need to supply your SS# on the new 2011 reimbursement form.
If any member knows they are going to change providers please complete the application process and turn in the proper forms to the Chairman at that time. This way the new provider can be approved and when it is time to switch it will be seamless and not result in loss of reimbursement.
