FAHRP Room Request Form (click here)
Request for Research Rooms
PI's need to submit room request form prior to ordering any EGGS that will require space in incubators or rooms. Waiting until eggs are set or until hatch occurs does not give ample time to organize housing and assure facilities are set up correctly.
Animal room request forms need to be submitted minimum 3 weeks prior to starting a trial. The request form should be sent to the primary animal care staff for the building(s) and Juliette Hanson. Please cc the entire animal care staff. (mccormick.10@osu.edu, Good.209@buckeyemail.osu.edu, Berlin.41@osu.edu, Myers.17@osu.edu, root.102@osu.edu, wright.1317@osu.edu, hanson.104@osu.edu).
A summary of your housing request will be sent out via email to confirm arrangements.
Include control animals and their space requirements on all room requests.
Animal care personnel will order all of the feed. Please do not order feed through individual labs. It is the responsibility of the PI/staff to inform animal care with 2 weeks notice of need for additional feed. If there are special needs beyond our standard diets, this should be placed on the room request form. Feed orders will be placed by the animal care personnel in charge of a particular facility.
For needs or adjustments during the trial, please contact the primary animal care personnel based on the facility. If you cannot find the primary animal care contact, please leave a phone message and a copy of emailed note in their office mailbox. Kingsly will be the secondary contact if primary contact is not available. If you do not hear back regarding your request within 24 hours please follow up.

Animal welfare or health concerns should be addressed immediately. Please contact animal care personnel or Juliette Hanson with any needs or concerns AND record your findings on the animal observation form.
Changes to experiments often have impacts outside of experimental design. Beyond considering whether protocol amendments are needed, PROMPT communication with animal care staff regarding any alterations to experiment is needed. This can help avoid waste or shortages in feed, anesthesia, and prevent lapses in care. We have been operating at high capacity of late so be mindful that changes to room requests may not be able to be accommodated.
We appreciate your help in getting rooms turned around rapidly. Once you are finished with your space, remove your biohazardous materials and inform animal care when the room has been hosed down and disinfected.
Record the date for the end of experiment and your initial clean out on the room cleaning log posted by each space (NIH and Isolation only)
